New patients who fail to bring the filled out health history forms with them to their appointment will have to complete them in the waiting room area. The needed time will come off your appointment time.

Missed Appointment Policy
Please be aware that there's a $150 charge for missed appointments without at least 48 hours notice. There's a long waiting list for appointments. If you miss your appointment, you are preventing someone from this list from being cared for and you'll also delay your care.
